New FMLA Forms Issued by the Department of Labor HomePrintE-mail

The Department of Labor (DOL) recently issued an updated Certification of Health Care Provider form (Form WH-380) and an updated Employer Response to Employee Request for Family or Medical Leave form (Form WH-381) with a current OMB Control Number (in upper right hand corner), which now expires on September 30, 2010.  The forms can be found with this link http://www.dol.gov/esa/whd/fmla/#form. Both forms are optional for employers to use.  However, each form states that “Persons are not required to respond to this collection of information unless it displays a currently valid OMB Control Number.”  If you are using the DOL’s forms, you should verify that the forms you are using are current.
